Nýlegar slysa- og atvikaskýrslur:

Alvarlegt flugumferðaratvik TF-FTZ (Cessna 172SP) og TF-JMB (De Havilland DHC-8-106) norður af Viðey

Árekstrarhætta myndaðist á milli flugvélanna sem voru að koma til lendingar á Reykjavíkurflugvelli. Rannsóknarnefnd flugslysa beinir í skýrslunni þremur tillögum í öryggisátt til Flugmálastjórnar Íslands og einum tilmælum til flugmanna.

Flugslys TF-HHX (Schweizer 300C) í Breiðdal

Stjórnmissir í flugtaki í Breiðdal á Reykjanesi er farþegi í ósjálfráða viðbragði ýtti ferilstýri þyrlunnar fram á við. Þyrlan valt fram yfir sig, stélkjálkinn (tailboom) brotnaði af og þyrlan hafnaði á hægri hlið með dautt á hreyfli. Farþegi og flugmaður komust óskaðaðir frá borði.

Fire at front spar of engine 3, shortly before landing at Zia Airport in Dhaka, Bangladesh.The airport fire brigate managed to put out the fire and the passengers and the crew disembarked the aircraft using the slides.

Flugslys TF-ABD (Piper Super Cub) á Melgerðismelum

Flugvélin ofreis í flugtaki, rak niður vængenda og brotlenti. Tveir menn voru innanborðs og sluppu þeir ómeiddir. Rannsóknarnefnd flugslysa beinir þeim tilmælum til flugmanna að halda brautarstefnu eftir flugtak og beygja ekki fyrr en öruggum hraða og hæð er náð.Flugvélin ofreis í flugtaki, rak niður vængenda og brotlenti. Accident N60842 (Piper PA-28-161) south-east of Vík í Mýrdal, Iceland

On February 21, 2008, about 11:39 coordinated universal time, a Piper PA-28-161, N60842, was ditched in the Atlantic Ocean, approximately 120 NM south-east of Vík í Mýrdal, Iceland. The airplane was being ferry-flown from the United States to Wick in Scotland. The NTSB investigated the accident and issued the report. The Icelandic AAIB (RNF) nominated an ACCREP to the investigation.

Accident N558RS close to Egilstadir Airport

The pilot of N558RS (Piper PA46) had planned to conduct a ferry flight from the USA to Finland via Canada, Greenland and Iceland. When landing at Egilsstaðir Airport (Iceland) the weather was considerably worse than forecasted. During final approach the pilot decided to perform a go-around due to low visibility. The decision was however made too late and the aircraft hit the ground approximately 700 meters short of the runway.

Accident N5030Q (Cessna 310) about 50 NM west of Keflavik

On February 11, 2008, about 16:11 coordinated universal time, a Cessna 310N, N5030Q, was presumed to have sustained substantial damage when the pilot reported ditching in the Atlantic Ocean, approximately 50 NM to the west of Keflavik, Iceland. The airplane was being ferry-flown from the United States to Bulgaria. The NTSB investigated the accident and issued the report. The Icelandic AAIB (RNF) nominated an ACCREP to the investigation.

Serious incident TF-FIV (Boeing-757-200) during cruise over Ireland

Enroute from Barcelona to Keflavik Airport, smoke was detected from a passenger armrest. The smoke came from headset audio jack and the In-Flight-Entertainment (IFE) system was subsequently shut-off and the flight continued to Keflavik Airport. The aircraft operator had recently installed new seats in the aircraft with updated IFE system. The investigation revealed that the audio jack had overheated due to design fault in the IFE system ground connection.
